I am currently working on a book about loving and caring for a parent (or grandparent) with Alzheimer's. Your experience and insight is needed.
I've attached files to help us get started. Please note that the Interview Release form has be to signed and returned to me. In addition, there are two separate forms: one for sons and daughters and one for grandchildren of Alzheimer's patients. These are preliminary forms. Please complete as much as applies to your situation and attach as much additional information (and pages) as you'd like.
